Please bring the following to your first appointment.

Driver's license
Your insurance cards
Physician referral forms, if required by insurance
List of prescriptions and/or over-the-counter medication, including dose and frequency
Information about your medical and surgical history
Recent x-rays or relevant records

Insurance and Payment Information

Michigan Allergy, Sinus & Asthma Specialists participate with most insurance carriers. Please call our office (248.932.0082) and ask our staff to assist you in determining whether we participate with your insurance and what type of allergy benefit you have. Please note that payment for all deductibles, co-payments, co-insurances, and non-covered services are due at the time of service. Master Medical payments are due at the time of service as well. We electronically bill these services so that reimbursement for Master Medical Services occurs within 5-7 business days.
